Athena Medtech is seeking an experienced Inpatient RN Case Manager to join our Case Management team. This role is critical in directing and coordinating the care of hospitalized patients to ensure optimal clinical outcomes, efficient resource utilization, and safe discharge transitions. The ideal candidate will bring expertise in acute care hospital discharge planning and demonstrate a patient-centered approach to care coordination.
Key Responsibilities:Serve as a core member of the interdisciplinary team to manage the patient's plan of care from admission through discharge.
Conduct comprehensive assessments to evaluate each patient's clinical needs, discharge requirements, and available covered services.
Collaborate with internal and external care providers to develop safe, timely, and cost-effective discharge plans.
Lead complex discharge planning meetings focused on minimizing unnecessary delays, reducing readmissions, and advocating for the patient's best interests.
Ensure continuity of care by coordinating with community resources, post-acute providers, and insurance case managers as needed.
Registered Nurse (RN) license required.
Minimum 2 years of recent acute care discharge planning experience (within the past 2 years) is required .
Strong clinical judgment with the ability to navigate complex discharge situations.
Excellent communication, documentation, and organizational skills.
Familiarity with utilization review and healthcare reimbursement guidelines is preferred.
